After making your initial appointment, you will receive an email link to set up an account with Patient Ally. Patient Ally is the portal where you can complete the intake form(s), communicate with your doctor, pay any outstanding bills, and view lab results. The intake form and your contact information can be filled out within your Patient Ally account. Please complete the intake form 24-48 hours prior to your visit.
An alternative method of completing the intake form is to download the form from the link below and upload it into a message directly to your doctor through Patient Ally or faxing it directly to us at 888-875-1198.
